CLINICAL SIGNS:
* loss of consciousness in a young adult, with the effort.
* sudden death.
* notion of recent infectious episode.
DIAGNOSTIC TESTS:
* ECG :
– often normal.
– QRS elongation> 0.11 seconds in the right precords whereas on the other leads, the QRS is normal.
– (QRS duration in V1, V2 or V3 – duration in V6) greater than or equal to 25 ms.
– Incomplete limb block aspect right on precordial straight lines (epsilon wave: small ST segment deflection).
Negative wave T in V1, V2 sometimes from V1 to V6.
– Ventricular extrasystoles with left delay.
* rhythmic assessment because risk of sudden death by ventricular tachycardia.
* echocardiography, cardiac scintigraphy, cardiac magnetic resonance imaging.
TREATMENT:
* hospitalization if doubt diagnosis.
– Establishment of an implantable defibrillator.
